Seminars and Support Groups

As part of our commitment to patient care and good clinical outcomes, we offer several educational opportunities. Patient seminars are designed to give prospective patients the opportunity to learn more about obesity surgery options such as the LAP-BAND® System and speak with both clinical professionals and patients.

Patient support groups are primarily geared for patients who have already had obesity surgery, but can be attended by prospective patients as well. They provide a critical forum for patients to share experiences, have questions answered, and gain encouragement from their peers.

Our patient seminars and support group meetings are held various locations serving West Los Angeles, Mid-Wilshire, and Orange County.


Insurance Verification

To determine if your insurance policy covers obesity (or “bariatric”) surgery, refer to the policy package that all insured people receive after they have paid their first premium, or if they have chosen a plan offered by their employer. Typically, there are two sections that describe the extent and limits of coverage. The first is usually called "What Is Covered" or "Covered Expenses." These are the healthcare benefits for which the company will pay. The other section is "What Is Not Covered" or "When the Plan Does Not Pay Benefits." In this section the company tells the insured which treatments they have to pay for themselves. You should refer to your insurance information to determine whether your policy covers surgical therapy for the treatment of morbid obesity.

Some policies will outright exclude bariatric surgeries. Others may have certain parameters around which bariatric procedures they cover and how much of the costs they cover.

The following is a list of items that are included in the $18,900.00 cash price set by Dr. Liu:

  • All facility charges for the date of surgery including:
    • Facility charges
    • Surgeon charges
    • Assistant Surgeon charges
    • Anesthesia charges & Anesthesiologist
    • All medications used during surgery
    • All supplies & equipment charges
    • Recovery room, etc.
  • Includes up to 3 fills in 6 months

All self pay payments must be in the form of a cashiers check payable to California Bariatric Management, Inc. no later then 10 days prior to the surgery date.


Submission Requirements

A Letter of Medical Necessity and weigh-loss history are necessary to obtain prior authorization for obesity surgery. A Letter of Medical Necessity states why significant weight loss is medically necessary for a patient and usually includes the following information:

• Patient’s weight (which should be 100 pounds or more above ideal weight or a BMI more than 40 or more than 35 with associated medical problems to qualify)
• List of medical problems associated with obesity, such as type 2 diabetes, sleep apnea, hypertension, etc.
• Number of years patient has been overweight (which should be at least five or more)
• Number and types of failed weight-loss programs attempted in the past

If you create a document or packet listing all your weight-loss attempts (self-controlled or medically supervised) and their results, you can substantially increase your chances of getting insurance coverage for the Bariatric procedure. You should include any commercial diets or medical records of your weight-loss efforts.


Appeals and Patient Financing

Patient Financing
When insurance reimbursement is not available, patient financing is another alternative you may consider. Please ask us about available patient financing programs during the patient seminar or your office visit.

Co payments
A co payment is a fixed amount of money you pay to the provider, facility, pharmacy, etc., when you receive services. Co payments are not the same for all services.

Example: When you see your PPO physician you pay a $20 co payment per office visit, and when you are admitted to a non-PPO hospital, you pay $100 per admission.

Deductible
A deductible is a fixed amount of covered expenses you must incur for certain covered services and supplies before your insurance starts paying benefits for them. The family deductible is satisfied when the combined covered expenses applied to the calendar year deductible for family members total the amount shown. Co payments do not count toward any deductible.

Coinsurance
Coinsurance is the percentage of our allowance that you must pay for your care. Coinsurance doesn’t begin until you meet your deductible.

Example: When you see a non-PPO physician, your coinsurance is 30% of your allowance for office visits.

Differences between allowance and the bill
Your “Plan allowance” is the amount they use to calculate their payment for the covered services. Fee-for-service plans arrive at their allowances in different ways, so their allowances vary. For more information about the allowed amount please call your insurance company.

Often, the provider’s bill is more than a fee-for-service plan’s allowance. Because we are not contract providers for any insurance we may bill you the difference.

This document, effective June 7, 2002, specifically describes how we handle your personally identifiable information and applies to all information that we collect and store in our databases.

If you are a patient visiting our web site we may collect the following types of information about you:

General information - Information such as your name, address, phone, and e-mail address to help your doctor contact you and bill your insurance company.

Information to verify your identify - Information such as your user name and password, in order to provide you with secure access. We may also verify your identify, or your status as a registered user, by means of cookies or sessions.

Surgical history information - Information about past surgeries, if any. Your doctor needs this background information to safely and effectively diagnose and treat your medical conditions.

Medical history and systems information - Information about medical problems or conditions that you currently have or have had in the past, if any. Your doctor needs this background information to safely and effectively diagnose and treat your medical conditions.

Medication and medication allergy information - Information about medications you are currently taking and medication allergies, if any. Your doctor needs this background information to safely and effectively diagnose and treat your medical conditions.

Social and family history information - Information about your social and family history. Your doctor need this background information to safely and effectively diagnose and treat your medical conditions.
